Salary: £37,147 – £43,010 per annum depending on experience (Pro-rata, based on 37.5 hours per week)

Job Summary
Are you a dedicated and forward-thinking Practice Nurse looking to make a real difference? Are you looking for a transition to General Practice Nursing. Lower Broughton Health Centre is looking for an enthusiastic individual to add to our supportive multi-disciplinary team of GPs, Practice Nurses, Health Care Assistants, Practice Pharmacists and Admin Staff.

Main Duties of the Job
As a Practice Nurse at Lower Broughton, you will be a key player in our clinical team. Your primary focus will be:
• Chronic Disease Management: Leading clinics for conditions such as Asthma, COPD, Diabetes, and Hypertension. Being able to assess, implement and evaluate individual treatment plans for patients with a known long-term condition or at risk of a long-term condition
• Cervical Cytology: Performing screening and providing advice on women’s health.
• Family Planning and Menopause care: Support and advise patients requesting information relating to family planning needs, and Menopause Health
• General Treatment Room Duties: Including immunisations and travel vaccinations (childhood and adults), and NHS health checks.
• Health Promotion: Empowering patients to manage their own health through education and lifestyle advice.
• Support vulnerable patients with learning disabilities and dementia
• Maintain disease registers
• Support nursing colleagues in maintaining infection control logs.

• Essential: Registered General Nurse (NMC), Motivated, Organised and excellent communication skills & empathy. Willingness to learn. Consideration may be given to nurse applicants who are experienced Community or secondary care nurses, particularly but not exclusively from those with diabetes or respiratory disease experience, who wish to move to primary care with training supported.
• Desirable Qualifications: Primary care experience, Independent Prescriber (or willingness to train), Experience using EMIS Web / Docman, Experience of Cervical Cytology & Chronic Disease Management, Proactive approach to QOF targets
