An excellent opportunity has arisen for the right candidate to join our Integrated Community Services Business Unit as a Heart Failure Nurse Specialist based at Fartown Health Centre.
Our Heart Failure Nurse Specialist team comprises of experienced specialist nurses, staff nurses and health care assistants.
The successful candidate will have an extensive cardiology background and be passionate about providing high standards of care and management for patients diagnosed with chronic heart failure condition. You will work in the community to support patients living in Huddersfield and also be a part of the wider Locala Kirklees Heart Failure Nurse Specialist service.
Key elements of the role will include:
• Providing management for avoidance of unplanned and unnecessary healthcare access related to heart failure condition
• Using and developing skills to provide specialist heart failure assessment of patients, assisting in the developing of self-management skills for patients and their carers’ and care plans for assessing the need for other services including palliative and social care and signposting as appropriate
• The role will incorporate advice and education within the primary and secondary care environment. The role will provide a resource for health care professionals and patients, providing both formal and informal education and training.
• Assisting in delivery of community clinics and home visits as needed
• Prescribing evidence based heart failure therapy within a heart failure formulary to optimise heart failure treatments to improve patient outcomes
